چکیده
Abstract Introduction The role of beta blockers (BB) post–myocardial infarction (MI) in patients without reduced ejection fraction the era invasive management is controversial results from observational studies have led to opposite results. Methods REBOOT an independent multinational pragmatic, controlled, randomized, open–label, with blinded endpoint adjudication clinical trial. study testing benefits BB maintenance therapy post–MI (either or ST segment elevation) discharged LVEF >40%. Patients ≥18 years old undergoing during admission and history heart failure (HF) are eligible participate. At discharge, they 1:1 randomized either receive (agent dose chosen by treating physician) no therapy. primary a composite all–cause death, nonfatal re–MI, hospitalization over minimum follow–up period 2.75 (minimum 2 years, maximum 3 years). Events adjudicated committee. Centro Nacional de Investigaciones Cardiovasculares Carlos III (CNIC) sponsor trial coordinator Spanish Centers while Istituto di Ricerche Farmacologiche Mario Negri IRCCS coordinates Italian participating network. Hospital for COVID–19 were also recorded since June 2020. Results currently ongoing 29 centers still actively recruiting. A sample size 8468 pts (728 events) was estimated provide power 85% detect RRR 20% (incidence control 10%) assuming 5% withdrawals. More than 75 35 recruited patients. first been included Spain October 2018, Italy January 2019. On December 31st 2022, number enrolled 1685. top three enrollers at moment are: Ospedale G. da Saliceto –Piacenza (483 pts), S. Luigi Gonzaga – Orbassano (183 pts) A.O San’Anna San Sebastiano Caserta (170 pts). patient enrollment rate almost stable all periods slight decrease COVID pandemic outbreaks. Network goal include 1800 April 2023. will become available end 2025. Conclusion REBOOT, other trials, robust evidence guide prescription after MI LVEF.

Evaluation of ejection fraction in patients with acute myocardial infarction undergoing percutaneous coronary intervention
Introduction: Percutaneous coronary intervention (PCI) is a reperfusion strategy to increase life expectancy and ejection fraction (EF) in ST-segment elevation myocardial infarction (STEMI) patients. We investigated the effect of location, severity, type of lesion, number and type of vessel involved and time of angioplasty on the increase in EF in STEMI patients undergoing primary PCI (P-PCI) a...
متن کاملExercise hemodynamics in patients with and without diastolic dysfunction and preserved ejection fraction after myocardial infarction.
BACKGROUND Left ventricular diastolic dysfunction (DD) is common after myocardial infarction (MI) despite preservation of left ventricular ejection fraction, yet it remains unclear how or whether DD affects cardiac hemodynamics with stress. METHODS AND RESULTS Invasive hemodynamic exercise testing was performed in 46 patients with a recent MI and left ventricular ejection fraction >45% and in...
متن کاملThe effect of eight weeks of moderate-intensity endurance training on myocardial capillary density, ejection fraction and left ventricular shortening fraction in male rats with myocardial infarction
Background and objective: Vascular obstruction and impaired blood flow and oxygen delivery cause a myocardial infarction and limit a personchr('39')s function. Therefore, finding a way to create new arteries and replace blocked arteries has always been of interest to researchers, including exercise physiologists. Therefore, this study was conducted to evaluate the effect of eight weeks of moder...
متن کاملProphylactic implantation of a defibrillator in patients with myocardial infarction and reduced ejection fraction.
BACKGROUND Patients with reduced left ventricular function after myocardial infarction are at risk for life-threatening ventricular arrhythmias. This randomized trial was designed to evaluate the effect of an implantable defibrillator on survival in such patients. METHODS Over the course of four years, we enrolled 1232 patients with a prior myocardial infarction and a left ventricular ejectio...
متن کاملSildenafil and diastolic dysfunction after acute myocardial infarction in patients with preserved ejection fraction: the Sildenafil and Diastolic Dysfunction After Acute Myocardial Infarction (SIDAMI) trial.
BACKGROUND Diastolic dysfunction is frequently seen after myocardial infarction and is characterized by a disproportionate increase in filling pressure during exercise to maintain stroke volume. We hypothesized that sildenafil would reduce filling pressure during exercise in patients with diastolic dysfunction after myocardial infarction. METHODS AND RESULTS Seventy patients with diastolic dy...
